Proper bandaging involves following these steps: cleaning the wound, applying moderate pressure, and wrapping from the distal end to the proximal end. Its main applications include securing dressings, supporting injured joints, applying pressure to stop bleeding, protecting the wound surface, and restricting limb movement.
1. Securing Dressings
The most basic function of a bandage is to firmly secure gauze or cotton pads to the wound surface, preventing displacement or detachment. When applying, first cover with a sterile dressing, then wrap the bandage in a circular or spiral pattern, ensuring complete coverage of the wound surface and smooth edges. This method is suitable for various cuts, abrasions, and postoperative incisions, effectively isolating external bacteria, maintaining a relatively stable local wound environment, promoting healing, and preventing secondary contamination.
2. Supporting Joints
For sprains or strains of the ankle, wrist, and other joints, bandages provide necessary external support and restrict abnormal range of motion. Using a figure-of-eight bandage better conforms to the joint's physiological structure, enhancing joint stability without hindering normal blood circulation. This physical support helps reduce pain, prevents further stretching of damaged ligaments, and creates favorable conditions for soft tissue repair. It is commonly used in the acute phase of sports injuries.
3. Compression Hemostasis
When bleeding is caused by the rupture of small blood vessels, applying even pressure with a bandage can achieve hemostasis. A thick layer of gauze should be placed at the wound as a pad, and then the bandage should be tightly wrapped to increase local pressure and block the blood flow channel. This measure is suitable for superficial venous bleeding or capillary oozing in the limbs. During the procedure, it is important to observe the color and temperature of the extremities to avoid excessive pressure that could lead to tissue ischemia and necrosis, thus achieving effective blood control.
4. Wound Protection
For patients with burns, scalds, or large areas of skin loss, using a bandage can reduce severe pain caused by friction from clothing and prevent the intrusion of dust and foreign objects. Choose a breathable mesh bandage or elastic bandage to maintain a moist wound environment and facilitate drainage of secretions. This protective mechanism reduces the risk of infection, alleviates patient discomfort, and is particularly suitable for the care of open injuries in areas prone to impact during daily activities, accelerating epithelial cell regeneration.
5. Restricting Activity
After fracture reduction or tendon suture surgery, immobilizing the affected limb in a functional position with a bandage can strictly restrict muscle contraction and joint flexion and extension movements. The effect is even better when used in conjunction with a splint, preventing re-displacement of the fracture ends or tearing of the tendon anastomosis. This forced immobilization is a crucial step in ensuring smooth connection between bone and soft tissue, and is applicable to various situations such as upper limb forearm fractures and lower limb leg fractures. Immobilization can only be removed after clinical confirmation of callus formation.
